1/12/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Cosby. Upon arrival to the home, Ms. Cosby appeared to be in an upbeat mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. She verbalized to QMHPA that her day was going well. QMHPA informed Ms. Cosby that this session was going to continue to focus on her ISP goals and the progress being made towards her goals. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Cosby current thoughts, feelings and emotions. QMHPA reiterated several relaxation/calming techniques Ms. Cosby could explore to assist with decreasing those feelings of stress, depression and anxiety. QMHPA reinforced conflict resolution skills such as listening and understanding what is being said before reacting/responding as well as walking away from altercations. QMHPA reinforced the importance of learning to communicate/interact/socialize with other in a positive manner without becoming angry/aggressive. QMHPA emphasized journaling, listening to music or recording music using her I-pod and continuing developing affirmations daily while also encouraging Ms. Cosby with replacing negative thinking with positive thinking. Ms. Cosby appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Cosby shared that she has not been into journal like she should because sometimes when she is writing how she feels her anger increase or her depression becomes more intense. Ms. Cosby was able to process when she was journaling it was easier for her to express her thoughts and feelings and even develop some solutions. Ms. Cosby shared that sometimes after reading what she writes she can see solutions and things she should or could have done. Ms. Cosby has agreed to make some changes in her life style that will help her increase her coping skills such as getting out more and being involved with her children inside and outside of the home. Ms. Cosby shared that she plans to make mental notes in her journal that she feels can help her decrease some of her anxiety, stress and feeling overwhelmed.
